Community Supports are essential services for Medi-Cal members, focusing on providing well-rounded care beyond a doctor’s office or hospital.At CenCal Health, we are committed to helping our members with their health-related social needs. These needs are often tied to issues like food insecurity, inadequate housing, and transportation barriers. Through Community Supports, we are working with our partners to positively impact health outcomes and overall wellness for the people we serve in San Luis Obispo and Santa Barbara counties.Starting today, we are expanding our Community Supports with our partners at Access Tlc Caregivers, Oxnard Family Circle ADHC, Independent Living Resource Center (ILRC) and Libertana. Here is what's new:- Asthma Remediation- Community Transition Services/Nursing Facility to a Home - Environmental Accessibility Adaptations (Home Modifications)- Nursing Facility Transition/ Diversion to Assisted Living Facilities(Residential Care Facilities for Elderly and Adult Residential Facilities)

Today, we honor Juneteenth. CenCal Health stands in solidarity with our communities as we commemorate this pivotal day in history, which marks the emancipation of enslaved African Americans. Today and every day, we are committed to promoting equality, justice, and inclusivity.To commemorate this day, CenCal Health participated in the NAACP Santa Maria - Lompoc, Juneteenth celebration held over the weekend in Old Town Lompoc. Congratulations to our local NAACP chapter for hosting this lovely event and kudos to all community partners and service organizations that were there sharing resources with Central Coast families
